PERFORMING RELEASE AND CONSENT AGREEMENT

I warrant that I am providing performing services as a professional, freelance, independent contractor. I warrant that all work performed shall be work for hire. This agreement constitutes the sole, complete and exclusive agreement between PEGGY PENNY INTERNATIONAL and myself and I am not relying on any other representation whether oral or written.

For valuable consideration, I, the undersigned, hereby irrevocable consent to the unrestricted use by PEGGY PENNY INTERNATIONAL, their successors and assigns, of my name, and any and all photography, video footage, sound recording and DVC computer images which have been taken of me during this contracted service. For all purposes use above, including without limitation, art, editorial, sales, advertising or trade shall be without further compensation to me.

It is explicitly understood that all original work performed will maintain copyright to the author of said original music. It is explicitly understood that raw footage and edited bodies of work of contracted performance will be the copyright of Peggy Penny International.

I hereby waive any right to inspect or approve the finished photograph, video advertising copy, or printed matter that may be used in conjunction therewith, or to the eventual use that it may be applied. I understand that Peggy Penny International is restricted to general R rating of any and all material produced or distributed by Peggy Penny International.

In connection with the foregoing, I hereby release and hold harmless PEGGY PENNY and Peggy Penny INTERNATIONAL from all liability pertaining to production and distribution of my performance in any and all photography, video footage, sound recording and DVC computer images distributed by Peggy Penny International.

I understand that my performance will be listed in the credits when applicable, using the name below.
